Dear Parent/Guardian:

In an effort to protect students from seasonal influenza, St. Luke’s Children’s is partnering with schools in the valley to help families’ access influenza vaccine for their children attending school. St. Luke’s will be offering an in-school influenza vaccine clinic for your student on Tuesday, October 29th. 

St. Luke’s Children’s will be offering injectable flu vaccine at no cost. We will be collecting insurance information for documentation purposes only. All paperwork must be completed in order for us to vaccinate your child.

If you would like your child to be vaccinated for seasonal influenza at our in-school clinic:

· Review consent and VIS (Vaccine information Statement) form in it’s entirely

· Fill out School Influenza Consent COMPLETELY. All questions must be answered for us to vaccinate your child.

· Return paperwork tomorrow, October 29th

· DO NOT return the form if you do not wish for your child to receive the flu vaccine.

*We apologize, but we are not able to vaccinate other family members.
